Living Well With Chronic Conditions

Chronic diseases and conditions such as hypertension, type 2 diabetes, obesity and heart disease can affect all aspects of life. Let MCHC provide you with the tools to allow you to better manage your health by enrolling in a free six week class called, Living Well with Chronic Conditions, an evidence-based workshop for people who have one or more chronic conditions.

Diabetes Prevention Program

Prediabetes is a serious condition affecting 1 out of 3 American adults—that’s 86 million people!  MCHC holds a Diabetes Prevention Program, which is a CDC-recognized lifestyle change program is a proven way to prevent or delay type 2 diabetes.  Contact [email protected] for more information on this program or how to enroll.

Breast Health Education

MCHC continues to educate women on Breast Health Education and the value of getting annual mammograms.   For more information please contact [email protected].

Alzheimer and Dementia Education

MCHC holds monthly Education workshops in multiple languages on:

  • Memory Loss, Dementia and Alzheimer’s
  • 10 Early Signs and Symptoms of Alzheimer’s
  • Strategies to Aging Well

MCHC Health Fair – Health Screenings and Services

The MCHC Health Fair will have health screenings and services include health checks, vision screening, blood pressure checks and blood sugar checks.

Partner agencies will be providing information and resources around a variety of health, education, and other services, including dental care.  Watch this space for more information on our upcoming Health Fair.
