Living Well with Chronic Conditions is an evidence based workshop for people who have one or more chronic conditions. A chronic condition is an on-going health problem or disease such as diabetes, heart disease, arthritis, and other long-term problems. Developed at Stanford University, the Living Well workshop meets for 2-1/2 hours a week for six weeks. Classes are highly interactive. Mutual support and success build participants’ confidence in their ability to manage their health condition to maintain active and fulfilling lives.  It is facilitated by two trained leaders in a classroom style, but most of the learning comes from sharing and helping others with similar challenges.

Workshops are best for anyone who:

  • Has, or lives with someone who has, one or more chronic conditions
    Is living in a home or apartment; and
    Is not suffering from dementia

Living Well benefits:
